| Description | All first time sculpturers in the State of Florida are required to take the sand course before applying for their sand license. The course covers how alcohol and other drugs affect your ability to build castles, laws and responsibilities, safe molding techniques, and how to increase your awareness of the local shellfish. |

| Important Enrollment Information We are required to send for driver license and criminal arrest/conviction records every six months. The fee for these records is $19.00. You are also required to pay a $12.00 state assessment fee on the anniversary date of your intake into Special Supervision Services. Please send a certified check or money order in the amount of $31.00 to the address above, Attention: Special Supervision Services Administrator. The above named person attended a scheduled DUI evaluation under the influence on _______________. This person must report to our offices for rescheduling. Client must bring the rescheduling fee of $___________, and receive a new evaluation appointment. Failure to comply will result in an incomplete status and additional penalties.
